I feel like Pinkie's opinion of her friends has become a self-fulfilling prophecy. Pinkie thinks they don't care about her problems at all, so she hides her real feelings from them and only shows off the most irritating side of herself, which drives her friends further away and further convinces Pinkie that they don't care about her.
I just think that if Pinkie gave her friends the chance to help her, she'd be pleasantly surprised:
