All it takes is one mistake and one misinterpretation of a situation to get a permanent scar from getting sucker punched. Realistically, charactpersople aren't perfect and do not have perfect judgment or perfect memory, not even Twilight, especially not over such a long time span.

Skills, including social skills and "reading the room," will rust away over time if not used.
 
Also, it could be a Starlight Glimmer situation, where Twilight _*cannot_* overpower her enemy with force and tries diplomacy instead... but it doesn't work out for her so well this time
