Spike has black bands on his arms the same color as Twilight’s blackened cutie mark.
 
For some reason or another, I don’t think Spike can leave Twilight’s side proximity-wise for too long otherwise something bad might happen to one or both of them. He seemed really distraught that Twilight’s little ‘date’ with the Mayor wasn’t doing anything to improve Cold Blooded Twilight’s “issue”.
 
(And unless the issue is something completely alien to the show’s lore, my biggest guess was that Cold Blooded Twilight’s soul was somehow fused with a Wendigo’s. Which would explain most of Twilight’s symptoms. But that’s just my guess.)
 
Assuming Twilight or Celestia aren’t secretly pure evil in this world, that the differences are just ‘more sex based society’ and ‘Canterlot and Twilight herself are more complicated’ my guess is those bands were put in place with Spike’s consent to help hinder whatever magical ailment Cold Blooded Twilight suffers from, by having Spike’s spirit and body act as a heat source to hinder the worst aspects. Which doesn’t work if he’s far away.
 
Hence why Spike’s going at full speed while its looking like a chore for him.
