According to something I read (which may have been made up by some fan) Caramel was going to be named Caramel Apple, and his caramel apple cutie mark was the correct one. The three horseshoes were a placeholder cutie mark that accidentally ended up in the final episode. Rather than correct it, the animators just rolled with it, and the horseshoes became his official cutie mark and the “Apple” part of his name was never used. But in some people’s fanon his real name is Caramel Apple and he’s part of the Apple family.
