I think you just broke records for that logical leap, boyo. Pinkie’s depiction in a single fanmade source is in no way the same thing as the dolls depiction in an official, canon source.
 
that being said, I think that this is just barely too far to be safe and agree with the semi-grimdark rating. But it’s a really hard one to call.

Tags on *booru sites are usually supposed to describe the actual content of a picture (aka “tag what you see”). And so it is with other semi-grimdark pics: explicitly shown blood, bodily harm, verge of suicide, insanity etc. This one looks out of place in that row. Also, if we tag basing on implications, we can tag every single Pinkie Pie pic as gore, because Cupcakes (a story) exists, features Pinkie and belongs to gore.  
That’s just an opinion, of course. I won’t touch the tags here anymore, if you truly believe it’s semi-grimdark, maybe it is.
