The reason(s) someone is into it varies, so I’ll just talk about myself since that’s the only frame of reference I understand the most. To start off, I’m not really into humans wearing diapers. I much prefer them on ponies where it looks absolutely adorable. I grew up with MLP (the show started when I was 10; been watching it since the beginning) and that was when i started noticing I had the fetish, and I thought humans in them looked weird so I thought about what it’d be like if ponies were in them instead. The rest is history.
 
And for me, there’s a slight BDSM part to it. I like the idea of being put into a diaper and being unable to take it off (pony hooves would sure make it next to impossible to do so!), even if I wanted to, and being forced to use it whether I like it or not, and all the while someone else may be watching. Yet, while I may be embarrassed/humiliated, I’m loving every second of it. I also like the idea of whoever’s watching being into it, confused, disgusted, being loving/caring, or a mixture of all of that.
 
So in short, I think for me it’s a mixture of being loved/cared for, and also some slight BDSM/humiliation aspects to it as well. I could add more to that but this comment was getting rather long, so that’s the gist of it.

as I recall when I asked the hottest he said he was considering it but he wanted to have an explanation for it and not just slap the Cutie Mark on

Why doesn’t Scootaloo already have her cutie mark in that? Since in terms of canon, she’s had it starting near the end of season 5.
