If you take a look at the first part, you can see that her pacifier never had straps. As for the reason why she’s not spitting it out, it might be the same as why she’s all flustered: she’s actually enjoying it (look at her trying to grope her chest with her mittened hands).

That’s what I came to suspect as well. I hope Applejack keeps her lead in the poll for the next one, because making it so Pinkie was behind it all along would be perfectly fitting.
