“Wax play” is melting wax (usually some with a low melting point so as not to cause scalding burns) onto skin. It can give deliciously hot sensation, and some enjoy the feeling of it being peeled off as much as it going on.
 
As for her anatomy, if she were an equine of normal horse build, it would require both shoulders to be dislocated, but I assume the artist is going with something more relaxed.
