Those both make potential sense
Personally, I figure that people simply think of his lair as nothing more than very ancient ruins, like we might treat some ruined temple or aqueduct from Roman times – they know the lair is there but they have no idea that anybody’s been using it in centuries, let alone that anybody’s using it right now.

Groger’s been sealed away so long, and is implied to be even older then they are, so it’s possible they don’t know about him, or believe he’s gone for good.
This could be mean either the sisters know about him potentially retuning or his lair just so happens to be an Equestrian landmark
Even without the icon, it looks like Groger’s lair there, a small detail we probably all missed.
