It’s Equestria Girls though, without magic and cutie marks to effect things, they might not have drifted apart in that world.
But in the show it’s implied that she never saw him again when he got his cutie mark until the events of The Crystalling. He wouldn’t have seen her goth phase.
I think the artist’s intentions are for them to be younger here since this is during Starlight’s goth phase, which presumably was when she was a teenager. So he probably just hasn’t grown it yet.
