Depends. If we’re talking someone being fat it’s not really my place to judge someone’s body shape or try to gauge healthiness of a random stranger. Healthy plus-size people exist and even if they aren’t they may be trying to fix that so it’s not really my place to comment, not to mention that they might have medical reasons for their weight, like thyro. (especially wouldn’t say something in public, and especially if the argument isn’t health but as i’ve seen people do unfortunately, about whether their body is acceptable by society, which telling someone they should feel like they have to unnecessarily change themselves if their appearance is undesirable to others is… stepping out of your lane to say the least and extremely rude and nunya.)
But kind of off topic.
In this scenario, underweight is different. While being over the average isn’t necessarily bad, being underaverage is more likely to cause immediate health issues even if it’s just a little underweight. If it were a stranger, though, who i’d never seen before (unlike sunny who is seen constanty by the townfolk, and they know she’s been healthy before) they might’ve gone through something like chemo or actually be a recovering anorexic and i wouldn’t immediately bring attention to it just because it’s mildly rude, and i do not know what they’re going through. if they show signs of being ano but not recovered i might reach out to them in a way that isn’t weird for strangers. (y’know just a little gesture of kindness or support for a fellow human being.)
With sunny tho and her history of being healthy before suddenly becoming sick, YES i would reach out even if just a little. She suddenly lost weight and is acting weird it’s only right to say something, (not call her out but just say something nice or try to support and express concern.)

You would think ponies would notice way before this stage. They don’t have baggy clothes to hide in. Chances are they did notice, but it’s not socially acceptable to call people out. So they kept quiet, then discussed it behind her back, but only talked to her when things became critical.
Is it body shaming to point out a possible health problem? How far does respecting personal choice go?
