Well, Doctor Whooves is (fanonically) based on The Doctor, but there are different ways of distinguish him, such as the hourglass Cutie Mark, blue eyes, no sideburns, and a different haircut. Maybe also slightly different clothes, and you can make him Asian\Hispanic\Middle Eastern\Indian\Native American\Pacific Islander\Black\Aboriginal\a gypsy\Inuit\Filipino\Thai\ambiguosly brown to differentiate.

the better question is: what is it if you base your pony on a real human then humanize it but don’t really make it look like the human it was originally based on?

Well for starting they do not have the same result as pooping is the end result of eating, secondly they involve completely orifices, thirdly one involves the intake of nutrients and the other the expelling of waste, and fourth they hopefully don’t take place in the same room.
