As said by @Background Pony #A406, I think it is meant to be a reference to the visual novel Saya no Uta.

It’s about this guy who sees reality as a twisted nightmare world, due to a side effect of an experimental brain surgery he underwent to save his life after a car crash.

The character we see here is likely the titular Saya. An extra-dimensional eldritch horror that the main character sees as a beautiful young girl, due to his condition.
