Ponyville isn’t really all that small  
 
Not all of the ponies attending were injured.  
Some of the injuries weren’t much more than a few bandaids.

Oh yeah….I almost forgot about the Everfree Forest…and the fact that Tartarus is only a few hours away…And the prime pranking spot of Discord…Geez, I feel sorry for the Ponyvillians.

Well it is ponyville, it has a habit of being a cozy little village that just so happens to be near both a eldritch encompassing Forrest of doom and the gates of hell a little a that ways… not to mention it being an absolutely safe and good-old fashioned every-pony knows every-pony town except when it becomes the accident and disaster capital of the kingdom!

…Honestly, I think the episode was really exaggerating with the ill ponies. Ponyville is a small town and if the episode was really true, a sizable portion of the town is injured, sick, and deformed…Yet, they seem fine in every other episode. {Cherry Berry got her hindlegs shrunk…That seems something like Poison Joke to me.}  
And if you notice, one of the injured ponies was Granny Smith’s father….I don’t think they were trying there.
 
It’s a good episode, but unless those sick ponies were coming from outside of Ponyville {And given some of their attributes, I find that highly unlikely} it was exaggerating.

Yeah,but apparently it isn’t very good if the population of ponyville with their modern hospital and various clinics still clamored in their bandaged bodies and makeshift wheelchairs rallied around two sale ponies and were willing try a bottle of strange fluid with the claim it could cure all their aches and ills for four bits.

Nah, Equestria has free medical health care.

I don’t think it’s safe to assume that in Equestria health care is the same of the USA.

Equestrian healthcare is the best in the world! But only if you live in Canterlot….
