Take the fifth cousin.  
Do the “twice removed” from them.  
Then do the fourth cousin from them.
 
Basically… severely estranged. With that much separating them (approximately five generations between the fourth cousins, six generations from the fifth cousins, and twice removed being another two generations’ difference. Or, between you and another person, you’d have to go back approximately 11 to 13 generations to find someone connected to you in the family tree.

I know what fourth cousin twice removed means, but what does “by a fifth cousin” mean?
