A sandwich is two pieces of bread/toast and what’s between them. A ham sandwich, for example, is ham between two slices of bread, not a slice of bread between two pieces of ham.
 
So a proper boob sandwich would be Celestia and Luna’s boobs between Twilight and someone else, let’s say Sunset Shimmer. Possibly squished together a little if Twilight and Sunset have gotten particularly close.
 
But anyway, that joke took up more of my life than I expected.
