Twilight does those things because she is spike’s guardian, and he is family to her. And he assists her in turn becuase it’s what he’s good at, but he’s not exactly under any formal obligation. He stays with her because she’s his family too.

What kind of ‘boss’ takes his employee to the doctor, give them baths, tuck them in, keep them from eating too much ice cream, take their comics when they stay up too late, wipes their tears and comforts them, follows them arround on quests to make sure they don’t get hurt, and has been caring for them since they hatched?
Spike is much more of a son/little brother and life companion who just helps out because he’s become good at it over the years.
