As Derpibooru is an image board, with new stuff being posted at a rate of ‘a whole heap’ every ‘not very long’ any image posted an hour ago is on the second page, any image posted a week ago is lost on page 50+  
To help people keep track of conversations, when other people are talking about an image more than an hour old or whatever, we have the notification system.
 
Click the number and you get a list of images with updates since the last time you visited that image.  
You may find that someone asked you a question and you’ve never answered because you’ve never been back to that image.

I never clicked it. I never had reason to because it meant nothing to me until everyone started paying attention to IT more than why I uploaded this image. To share my shock at seeing so much of the same thing on the front page.

About the 233 thing, I had no idea what that number was, and didn’t know it was for notifications. It’s not like it’s incredibly obvious like Steam’s green notifications button is.
