They appear to be based off of the Marked Men from the Lonesome Road DLC.
The backstory of them is that a NCR army was squaring off to fight a Legion army in the Divide when they were both hit by a huge radioactive sandstorm. The those that didn’t die from having their skin stripped off became ghouls but were unable to leave the Divide because they’d die without the high radiation levels. The two sides united due to the shared CONSTANT HORRIBLE AGONY and will brutally murder and probably eat anything that trespasses. Note that they aren’t feral, they’re still able to reason and use weapons, they’re just insane and murderous.
Actually NCR Troopers in New Vegas wears helmets like that. Actually, the implications are unpleasant when you think about it.
