Yay, I found it. The story is Hippie Hat Brain Parasite, a fairly early thing by some hack named William Gibson. “Just neatly nibbled off at the… hatline. Kinda scarred, in there, healed over, grayish-pink. I saw where the hat had had its claws in, kinda puppet trip…”. Had it pegged as PKD initially.
