I was browsing the pony merch at a department store when I spotted this Twilight Sparkle figure whose Spike had its head tuft out. I’m guessing this is removable by design so as to put hats on him or something. Right?

No, look by Spike’s feet, that little green thing, it is the part that goes into that hole. Part of his head either fell off in shipping or it was taken out of the box with little care.
Nah, you can see that the packaging is molded to fit it, and it’s held in place with a cord.
