He’s a defector. Even if his assessment of her morality wasn’t entirely accurate, he clearly felt strongly enough about it to leave his own kind.
Actually, three options are available. The first is that she actually gives birth to all changelings; unlikely. The second is that he is either one of her only sons or her only son, making him a prince; more likely. The final option is that she is the type of queen that treats her subjects as her children, making her subjects feel like they are indebted to her; least likely, but interesting.

Maybe he’s copying someone else’s voice. Copying ponies is sort of how Changelings operate, so what’s to stop him copying some random pony’s voice to talk with Spike?
Maybe she thought it unnerved the ponies, but didn’t think she needed to at other times.
