My headcanon: Caramel and Western Caramel (not pictured) are identical twin brothers, who are also fraternal twins of Big Caramel and Female Caramel. The quadruplets are the children of Doctor Caramel (aka Doctor Stable) and some earth pony mare.

Pegasus Caramel is just some pegasus who happens to look a lot like them, but isn’t actually related.
