Fluttershy had her name changed after she got her cutiemark. It’s actually very common, since ponies are confused and worried by mismatched names. Or just embarrassed by them, as “Lulamoon” and “Sweet Drops” can attest.
 
Rarity used to be called “Kegger” by her dad. And Applejack was once “Tequila”. (They were really hoping she’d buck the apple tradition and get a cactus cutiemark, but no such luck.)
 
You don’t want to know what Fluttershy used to be called. Let’s just say her parents had very, very high expectations.

I’m pretty sure that this design is intentional… but like Chumpy said, it’s because the character designers know the story.
 
Pony names get pretty confusing if you think about it in real-world terms. How would Fluttershy’s parents know she would grow up to be shy? And how would Scootaloo’s parents know that she would ride a scooter?

Need I point out that Twilight is a fictional character designed by humans who already had the entire story drawn out and likely put that design there intentionally to signify her destiny? I mean, it’s not like she just spontaneously developed the mark on her own during development….

That’s neat but kinda creepy. Like, how it was her destiny to be friends with the other elements. It’s like a god saying I didn’t really have a choice who I can have as friends when I was created.
