When startled they release the toxin into the surrounding water, multiple reports of Dolphins using this to achieve the equivelent of a drug high by harrassing a puffer fish have been documented, and some do indeed have spines that can poison.

Actually, many pufferfish are poisonous, just not to the degree they are in fiction.
