Because, as Meanlucario said, Trash and Sunny are the same mare.  
The only difference is that Sunny is clean with a bright and vibrant color scheme, whereas Trash is so covered in dirt, filth and grime that you can’t see what her natural colors are.
 
However, this particular pic (and a few others) are set in an AU in which Sunny never became Trash, so she became the business mogul that her little sister, Lime, became in the main verse, and Lime works for Sunny here.

I just realized Lime could be a really disturbing horror monster, like some fucked up, super fast, double-jointed witch with pale skin, long, spindly limbs, and clawed fingers living in the forest where she feeds off the blood of children that wander into her hunting grounds.
 
Like, she’d stalk them through the brush on all fours, and you’ll know she’s getting closer when you hear her bones clicking - but that’s when it’s too late. Then she digs her dirty claws into you and rips your throat out, digging through your remains and bathing itself in your blood like a frenzied animal

I wasn’t going to say she was mentally unwell until I found out she paid the health instructors to leave her alone. There’s being too busy and stressed to eat, and then there’s actively refusing the advice and instructions from experts. That’s like saying, “I’m not obese because I have an eating disorder. I just had some doctors come over, heard them tell me to eat less because I was going to die soon, and then paid them to leave me alone and kept on eating cake for breakfast. It’s just the stress.” That fatto has to stop making excuses.
 
Fucking take your lunch break to mcdonalds and stop trying to justify mental illness, drawing of a fictional pony who can’t hear me and isn’t even alive. I got all the way to the end before I realized how pointless this rant was. But I never backed down from a trainwreck before, so why stop now?

Lime mightn’t look it, but she’s pretty healthy for the most part.  
Sunny just wants her sister to look a little more… conventionally acceptable.

She’s trying to get back down to her birth weight.

Poor Lime, this can’t be healthy for her, she’s probably not feeling it now though.
