Well, to be fair, they’re generally only drawn as above, just a discoloured protrusion. The point of explicit is that it’s sexually explicit. The “sexually” part is implied. Meaning if it’s not related to sex (i.e. cock, balls, vagina) then it’s not explicit. Suggestive generally relates to implication of being sexually explicit (i.e. a bulge) as it implies something is sexually explicit. Safe, in this case, refers to natural bodyparts. It’s not sexual, it’s natural. It just sits there.

To those of you removing the safe tag and adding explicit; sheaths are not explicit.
Quoting from the explicit tag description:

→Genitals (penis, vagina/vulva, balls, etc.) including any detailed visuals of them, note that sheaths are like any normal body part (safe until sexualized)
