The source offers more background on the picture and says that Luna is very protective of Sombra, because she knows that ponies with the kinds of pasts that they have don’t have it easy. If you clicky the source there’s other stuff too, but that’s the short of it.

Unicorns must attend mandatory horn safety classes to reduce the chances of accidentally goring another pony with their horns. Something they must always be mindful of. Having a giant spike on the top of your head may seem like a great idea, but it’s a real mood killer when you always have to be mindful when physically interacting with others.
