Just looks like that changelings carapace is filling the space where the ponies internal organs aught to be.
A fatal dog pile of sorts.

Could just be that the carapace contorts upwards, like if you pull in your stomach, it just looked rather crushing.
Aside from that It’s drawn rather well. nicely done.
