Lets not forget the fact that Caramel later showed up as a mare in Appleloosa, making the way for a lot of jokes about him getting a sex change to please mac.

This was early on in the show’s run, at the time, there were only a few male characters with any characterization (Caramel was characterized as being absent-minded and was implied to be a little unconfident), and he and Big Macintosh were apparently working together during Winter Wrap Up. Since Spike is a child and all the other characterized stallions (Hoity Toity, Prince Blueblood, etc.) never interacted, Big Mac and Caramel was the only natural gay ship at the time.
 
The fandom was also tiny at the time as well, so the ship caught on since small things like that affected larger percentages of the fandom than they do nowdays.

It’s partially because Caramel x Big Macintosh was such a well supported ship for a very long time.
