Funny enough, I just found the next page to this on Pixiv, and posted it here: https://derpibooru.org/397155
So yeah, Sweetie Belle did indeed give Fluttershy…something or other. And now we know Applejack’s role.
I think Sweetie Belle decided that, for Fluttershy, that much smiling counts as a laugh, and she’s about to get punished.
I’m guessing that Sweetie Belle’s actually giving Fluttershy something, probably as part of the challenge.

In this case, yeah. You can usually tell by how it’s written. Most often, it’s written vertically, which means the panels are right-to-left. Here, though, it’s written horizontally, so you read it left-to-right like a Western comic.
