I’m OK with that but why not both?
It’s clear that she spends a lot of time at the spa and spends a lot of money there (possibly in the form of large tips. You know, generosity…) so it would be normal that the spa twins would be more willing to give her better service…
I think the point of the spa scene was to show that Rarity likes being pampered and paid attention to, while Fluttershy isn’t so “high maintenance”. It sets up the plot point later on about Fluttershy hating all the attention she gets as a model, and Rarity being jealous that it’s not HER getting all the attention.
Well, DUH! She has a secret horn-filing fetish.

I interpreted that whole spa sequence as Rarity simply getting preferential treatment.

Well, that’s specifically a horn filer, which she doesn’t need on more physiological grounds.
I thought that was the point of the scene was that she has less stress than the others, so she doesn’t need much work.
