With all these confusing emotions inside of him, Smokin’ began to grow depressed. His performances stopped, and he would rarely ever talk to anyone. After some time and constant prodding from his parents, Smokin’ finally told them how he felt. Of course, they were surprised, and slightly concerned. The conversation went a little something like this:
“So… are you a colt cuddler then?” Candle Wick asked.
Smokin’ Mirrors sighed and glanced away, unable to meet his father’s eyes. “I… I dunno dad. It’s just… I don’t feel like a stallion,” he murmured.
“What ever do you mean, dear?” his mother asked.
“I mean, well, I mean that I feel like… like a mare,” Smokin’ replied, staring down at his hooves.
Anywho, is parents were very confused at first, and Smokin’ told them about a surgery in Manehatten he’d heard of that would allow him to essentially become a mare. And although they weren’t exactly keen on the idea, his parents loved him enough that they would support his choices. Two weeks later, he boarded a train to Manehatten with all the money he’d saved up over the last few summers from working on one of the local farms. And within a few months, the pony returned to the village; only now, she was a mare. Though her parents were tentative at first, they eventually embraced their new daughter full-heartedly. Unfortunately, her brother was a different story entirely. Dream Catcher had grown up idolizing his elder brother, and when Smokin’ Mirrors came back as a mare, Dream resented her. The younger sibling would encourage the bullies in the town to focus their wrath on her, causing her to constantly come home bruised and bloodied. It was all too much for Smokin’, and so she bid her family farewell and moved away. Currently, she is a resident in Ponyville.

Transhumanism is right around the corner and the singularity has been predicted by some to be made within our lifetime.
But yes, cosmetic change is basically all any transsexual really hopes to get, and it’s typically more than enough for the Time.
It also gives me some interesting thoughts about designer babies. It could be possible within this century to determine how many limbs your baby has, or even have transgenic babies using genes from other species. Maybe you could even have babies with multiple pairs of functional arms, or two dicks.
The amount of ethical controversy would be ungodly, though.
If you can create and attach new complex tissue structures it seems reasonable that organs would be possible to, organs just being tissue compounded. That would mean giving an MtF ovaries and the components to use them would be possible.
Even if that wasn’t so for whatever reason, with that level of control to augment a body you’d still have a reassignment process that’s unthinkable in detail and complexity today.
Meh, that’s just cosmetic stuff.
Maybe there’s no reason to touch something on a genetic level when you can make it just fine with external sculpting and changing tissues?
If you think changing the very chromosomes of a body is somehow just as superficial as restructuring tissue and bone, I’d say you’re the one who’s stretching things.
Functional Wings would require a complete remapping of bone, muscle, nerve, and circulatory systems and structures around the spine, which is far more delicate than any sexual reassignment surgery would need. Calling that ‘merely’ anything seems a bit out of touch

we’ve seen Trixe pull off pinkies muzzles and twilight gain wings. adding and removing complex tissue and structures is basically all you’d need to be able to do.

It’s always been that way in FIM, or have you forgotten how impossible it was for Twilight to just give Applebloom a cutie mark? Messing with the destiny a pony was born with by magic is tantamount to mad science, or playing God.

I have to admit that since we watched trixe do the impossible, that kinda makes it seem just…implausible at worst, rather than completely and utterly undo-able.
They also have a time-travel spell. honestly a gender bending spell has to be a step down from that.
Concerning the ruse of various forms of magic that Twilight performed in order to fool Trixie, Trixie exclaimed: “But… how did you do these spells? Nopony can do those spells!”, to which Twilight Sparkle unambiguously said, “You’re right.”
It was earlier in the episode that the castings Trixie was doing were regarded as simply being of a very high level.
As to any speculation regarding other means or what-have-you, I have nothing at all to say.
I thought Twilight said “only a high level (?) unicorn can perform these spells!”, to which Trixie responded “And I’m a high level unicorn!” Was that about something else? It’s not as if I remember every episode word by word.
And perhaps it can’t be done by magic, but perhaps surgery? I mean, she couldn’t be slimmed down that much, but still.
She can alter people’s bodies in dozens of ways, but a gender change is just out if the question.
