The reason why Duke didn’t die in the G.I. movie is was because of the fan back lash of Optimus Prime’s death in the Trasnformers movie, they choose not to make kids cry if Duke dies in the movie just like what they did to the Trasformers movie when they make fans cry and mourn over the death of Optimus Prime by replacing Duke’s death with a coma to fit perfectly in the final film. Also, at the end of the film, Doc radios General Hawk that Duke has been fully recovered from his coma.

To be fair, they weren’t originally planning to bring Optimus Prime back to life, and only did that, because kids/fans were so upset by that. Hasbro & the makers of the original Transformers cartoon had no idea Optimus was such a beloved, and iconic character for the series at the time.

Only one I could see happening maybe would be the CMC thing. Funny thing is, now that their voices are pretty much grown-up, or in their teens now, Pipsqueak is the only “kid” character in the show with a genuine kid voice actor. (Granted, they replaced him, after the old one hit puberty, and has a man-voice now.)
