That only applied to religious crosses, and even then some of them slipped by.
Nah, that had to do with Nintendo of America’s policies in the 90’s (they wanted nothing to do with crosses of any kind). Later in Super Smash Bros Melee they forgot to remove the Red Cross from the hospital in the background of an Earthboubd stage.

I’ll just headcannon that in her previous appearances she had been filling in the cross for cosmetic purposes.
Plus I might just headcanon this change to be a result of her acquiring more medical knowledge. Like maybe she’s moved from regular nurse to head nurse at the hospital.
