The Gaming Historian did a good video on it. Long story short is that it is an urban myth, probably stemmed from similar yet unrelated circumstances that happened in arcades at the time. Supposedly, it was an arcade machine that was used by intelligence agencies as a means of brainwashing or mind-controlling the players who used it, most of whom got sick or even died whilst playing it.

Basically, the rumor is that it was an experiment of the US army.  
See the extreme effects of subliminal messages on videogame format. A fan recreation was made out of the rumors.  
Gameplay wise is a generic asteroids arcade of the 80s. The problem is about the content: fast and flashing lights, loud noice, plus the subliminal messages to become another system’s “good boy” (during the game, fast words show up saying things like “buy” or “conform”. The most alarming is “always obey us”).  
After the game session, the players said that they have strong headaches or earned insomnia with amnessia.  
So far the only evidences are just pictures of the arcade machine and the testimonies.

Ohohoho, that’s a big one. It’s hard to say a lot in one comment, but to the best of my understanding, it’s probably one of the most controversial videogames, not because of content it possessed, but because it’s existence. Some said it only existed in a small part in an area of Oregon, some say it never did.
 
That game breaks the rarity scale that a good majority don’t believe it ever existed, and those that did have no means of proving it.
