The actress pony for Fluttershy was actually an earth pony as well. The casting is still highly controversial. Rarity was played by a trans mare.
 
I actually have a question about the existence of trans ponies in Equestria.
 
I mean, I’m sure they exist.
 
But they have literal magic to switch someone’s sex, completely and perfectly (as seen in the magic duel between Trixie and Twilight; Trixie wasn’t shocked by Applejack’s “miraculous transformation” into a stallion, she was just shocked that Twilight could pull off the spell, just like that) It’s not a “doing the best we can with the knowledge we have” situation like on Earth.
 
Even if the sex-change spell is extremely difficult and complex magic, I’m sure it can be taught and studied, and there are doctors who can specialize in it.

Really? Because a lot of us got the opposite impression there - that it was an example of magic no unicorn, not Twilight Sparkle, not even one wearing the Alicorn Amulet could do. (Now that doesn’t necessarily rule out an alicorn being able to cast it, but it doesn’t rule it in either.)
 
Of course, pony magic isn’t the only magic out there, even if they are the greatest spellcasters. I’m sure the wishing flower from Growing Up is Hard to Do could transition a pony or three, and there might be other sources like a magical spring or something. But I don’t think it’s as simple as the flash of a horn - rather, something worth questing for.

There’s also the question of if it’s a complete change. Does turning a stallion into a mare create a uterus and ovaries? Can this mare reproduce? Is the spell permanent or require upkeep? Is it highly complex, requiring intense concentration and power? How does one practice such a spell? What happens if it goes wrong? Can it be corrected?  
If growing hair is practically impossible, changing sex is likely even more difficult.

In my universe, I make sex changing spells and everything related to them veeeeery tricky to pull off and only truly capable with some chance of working 100% with only the most powerful and professionally trained unicorns.
 
It’s not something that can be done casually, or lightly, and it is very risky.

If magic can do this, it can do just about anything you can imagine. Of course, this was an accident, too, and may not be 100% repeatable or give 100% predictable results. (“Results may vary. Consult your unicorn if symptoms persist.”)

It’s not something that can be done casually, or lightly, and it is very risky.
 
Well, the alternative is a complex and risky surgical procedure which leaves one permanently unable to reproduce (and in the case of female-to-male, can’t establish normal sexual function). It’s only logical that if doing it magically were possible at all, there’d be a fair amount of research into developing that into a safe and effective alternative to having one’s naughty parts cut up.

Changing the external appearance is easier than changing the internal structure. You can make a frog look like an orange, but it’s still a frog.
 
Effective gender transitions are about as difficult as effective species transformations. Unless you have an alicorn or a specialized artifact, good luck.

I use the same dodge I avoid healing magic with - ponies’ own magic slowly dissolves any unicorn spells cast on them. (You might be able to transform yourself, but what are the odds you’ll be able to learn it?)

I’m probably late for the thing I am going to post, but once i did saw a fanfiction that said that the changelings were normal ponies that had been horribly experimented and mutated grossly (Imagine chunks of your body being torn and ripped apart and your love being drained, the latter worse than you think) by Discord during the time he was the absolute ruler of the land, just for fun, then they were hunted like wild animals and forced to slavery after Celestia and Luna came into power due to their physical appereance and their need for love to survive. And that Thorax (along with the rest of the changelings) changes after sharing love were actually a disease created by Celestia on a last ditch effort to avoid their win.
 
On a slightly unrelated note: I’ve seen headcanons that said that Pinkie and Discord are actually humans transformed into ponies and a mishmash of stuff respectively, the first one by Starswirls the Bearded and the second one by an unexperienced tyrant Celestia.
