Personally, I favor the idea that she can never be truly sane. That her cutie-mark represents insanity, such that any cure would send her into Cutie-Mark Failure Insanity Syndrome.
 
Her treatment consisted of using mind control spells to drive her insane in a just as “nutsy” way as she already was, but less debilitating. THEN she was cured of acting like a dog/thinking she was a dog.
